Welcome to the Pedro Menendez High School Clinic site. Here you will find Medication Authorization Forms and Treatment Care Plans for the 2023-2024 school year. Be sure all forms are completed in full and signed by your physician.
Completed forms and medication (in a properly labeled container that includes the students full name and current prescription) must be dropped off to the clinic by a PARENT. The clinic will open Monday, August 7 at 9:00 AM.
