Welcome to the PMHS clinic website. Here you will find required Medication Authorization Forms and Treatment Care Plans for the 2025-2026 school year. Be sure that all forms are completed in full and signed by your physician where indicated.
Medication that will be administered by the nurse along with the completed forms must be dropped off by a parent. Prescriptions must be current and properly labeled by a pharmacist. The clinic will open for your convenience beginning Wednesday August 5, 2026 at 9:00 AM. Please call ahead or email me if you intend to stop by.
