Here you will find required Medication Authorization Forms and Treatment Care Plans for the 2025-2026 school year. Be sure that all forms are completed in full and signed by your <\/span><strong style=\"font-size: 16px\">physician <\/strong><span style=\"font-size: 16px\">where indicated.<\/span><\/p><p>Medication that will be administered by the nurse along with the completed forms must be dropped off by a parent. Prescriptions must be current and properly labeled by a pharmacist.
